Preparing for Your Visit
Please have your child's pediatrician or primary care provider send a referral, including clinical records and any diagnostic and laboratory test results.
Please note that some insurance plans require pre-authorization and/or physician referral before your child’s appointment. Your copay is listed on your insurance card, and will be collected when you check-in. Please contact your insurance company in advance if you have any questions.
It’s very important that we know exactly which medications (prescriptions and over-the-counter) your child is taking. Please bring all of your child's medications in their original containers with you to their appointment.
